Medicaid Home Care Programs by State: A Quick Guide for Agencies

Every state funds Medicaid home care a little differently — different program names, different waivers, different rules for who qualifies and how agencies are paid. If you’re serving (or expanding into) a new state, the first step is knowing which program the families you want are actually enrolled in. Here’s a quick map for the states Adult Care Network covers.

This is a general orientation for agencies, not legal or eligibility advice. Program names, rates, and rules change — always confirm the current details with the official state source linked for each program.

What this means for lead generation

Two agencies in the same state can serve very different programs, which is why “matching” matters more than volume. A lead is only useful to you if the family is in a county you cover and in a program you’re enrolled to serve. That’s the whole point of pre-screening: confirming program fit and service area before the lead ever reaches your portal, so you’re not paying for contacts you can’t actually take.

Adult Care Network screens each family against a written standard and routes them only to matched agencies — no more than three per lead — for a flat fee per qualified lead, never per enrollment.
